NCCD Consent Form
Attached is a document from the Catholic Education Commission of Victoria (CECV) about the Nationally Consistent Collection of Data on School Students with a disability (NCCD).This is to inform parents of students who are receiving adjustments and accommodations at school because of a medical condition or disability, that we will be using the names of students in this data submitted to the CECV.
If you do not consent for this to occur, then a number will be used instead of your child’s name.
To do this, you must fill in the NON CONSENT form attached and return it to school by Friday 11th August.
If you do not have any concerns, then you don’t need to do anything.
